Step 2: Complete An Admissions Inquiry

Please complete the form and we’ll provide you with more information about how Heartlight can help.

Step 3: Complete and Submit The Application & Medical History Form

Please complete and submit the (a) Admissions Application and (b) Medical History form. Both can be completed online. The $300 application fee is non-refundable. Should your child be accepted into the Heartlight program, the application fee will be applied to the first month’s tuition.

The Adolescent Application can be downloaded and completed in-person by your child.
